The Transition Care RN (TCM) ensures your needs and preferences to care are being met and we work with your team to make sure you meet those needs over time.
We lessen the burden and help provide a smooth transition for you from the hospital back to your home , educating you along the way about changes that may have occurred.
The TCM nurse creates your individual care plan, educates you on changes in your medication, wound care and/or connects you with services to meet you social needs.
We identify and resolve potential issues to prevent non-emergent hospital visits to avoid unnecessary hospital readmissions.
We communicate with you, your primary provider and your family to make sure your progress aligns with the anticipated goals.
